Required Pre-Qualification Documents

Interim Waiver of Lien and Release of Supplier – Use this form when you have unpaid or open invoice(s) with your supplier/vendor, including any equipment rental company, service provider (such as temporary labor/work force). Also use this form for supplier of Owner Material Purchase (for Tax Exempt project). This must be signed by the supplier.

Partial Waiver of Lien and Release of Sub-Subcontractor – Use this form if you hire your own subcontractor to perform work for you and you have unpaid invoice(s) or open balance with your subcontractor. We call your subcontractor as “Sub-Subcontractor” as stated on the document and must be signed by the Sub-Subcontractor.

When submitting retainage billing at the end of the project, the following documents are required:

Final Waiver of Lien and Release of Sub-Subcontractor – Use this form only when you have fully paid your subcontractor and must be signed by your subcontractor.
